Serving on our Board is more than a governance role — it is an opportunity to make a tangible difference in the lives of thousands of Canadians each year.

As a Board Member, you will:

• Help shape the strategic direction of a national, patient-led organization
• Contribute your expertise to advance meaningful, high-impact initiatives
• Expand access to critical support, information, and advocacy for patients and caregivers
• Support and advance fundraising and resource development efforts
• Collaborate with a passionate and committed group of leaders dedicated to change

Whether your background is in healthcare, governance, fundraising, communications, policy, or lived experience, your perspective can help strengthen our work and broaden our reach.

We are seeking individuals who:

• Are passionate about improving patient outcomes and experiences
• Bring diverse perspectives, skills, and lived experiences
• Demonstrate a commitment to collaboration, accountability, equity, and impact
• Are motivated to leverage their networks and influence to support the organization’s growth

We are particularly interested in candidates who can support our priorities in areas such as health equity, patient engagement, system navigation, awareness, and sustainable growth.

This nomination form is designed to help us better understand your background, experience, and interest in joining our Board. It should take approximately 10–15 minutes to complete.

All submissions will be reviewed by our Nominating Committee as part of a thoughtful and structured recruitment process.
